Patient Participation Group

We have formed a group to promote the views of all the users of the Eastbury Surgery. Members are patients at this practice.

Our aim is for you to get the optimum service here.

Anyone over the age of 18 who is a registered patient of the Eastbury Sugery can become a member.  We do ask that you are enthusiastic about the Practice developing and delivering its services to the community.

You do not need to have a health care background.  Members of minorities are especially welcome to enable us to reflect the diversity of the local community.

We are also very keen to forge links with the young people in our community and would welcome hearing from any schools or groups who would like to be involved.

If you are interested in being a member or supporter of the PPG or would just like to find out more, please contact the Practice Manager, Rupal Nathwani on 01923 845 945.or ask for a registration form at Reception.

Virtual Group

We are also proposing an email service for people who are interested but have work, children, or other commitments. The group needs to be representative of all the people who are looked after here.

We will ask the members of this representative group some questions from time to time, such as what you think about our opening times or the quality of the care or service you received. We will contact you via email and keep our surveys succinct so it shouldn’t take too much of your time.

You can download the sign up form as a pdf document, print it out, complete it and return it to the practice.

We will be in touch shortly after we receive your form. Please note that no medical information or questions will be responded to.

Many thanks for your assistance
